Using MediSave for chronic conditions
Under CDMP, patients with a covered chronic condition can withdraw MediSave to pay for outpatient treatment at MediSave-accredited providers, including all polyclinics, public hospital specialist clinics and accredited or CHAS GP clinics.
- The standard withdrawal limit is S$500 per patient per year.
- Patients with complex chronic conditions (more than one covered condition, or one with a recognised complication) have a higher limit of S$700 per year.
- A 15% cash co-payment applies to each MediSave claim.
- Since 1 February 2024, the 15% co-payment is waived when treatment is at the patient’s enrolled Healthier SG clinic.
Which conditions are covered
CDMP currently covers around 23 chronic conditions. These include the most common long-term illnesses managed in primary care.
- Diabetes and pre-diabetes, hypertension, lipid disorders (hyperlipidaemia), stroke and ischaemic heart disease.
- Asthma, chronic obstructive pulmonary disease (COPD) and allergic rhinitis.
- Mental health conditions: schizophrenia, major depression, bipolar disorder, anxiety and dementia.
- Others including osteoarthritis, rheumatoid arthritis, osteoporosis, gout, Parkinson’s disease, epilepsy, benign prostatic hyperplasia, chronic kidney disease, psoriasis and chronic hepatitis B.
CHAS card tiers and eligibility
CHAS subsidies sit on top of MediSave use and are available to Singapore Citizens. Your tier is set by household monthly income per person, or by the Annual Value (AV) of your home where there is no income.
- CHAS Blue: income per person of S$1,500 and below, or home AV of S$21,000 and below.
- CHAS Orange: income per person of S$1,501 to S$2,300, or home AV of S$21,001 to S$31,000.
- CHAS Green: above those thresholds; open to all citizens.
- Pioneer Generation and Merdeka Generation cardholders receive enhanced subsidies regardless of income.
What CHAS subsidises
CHAS subsidy amounts depend on tier and on whether the visit is for a common illness, a chronic condition or dental care.
- Chronic disease visits are subsidised per visit up to an annual cap, with higher subsidies for the lower-income tiers and for complex conditions.
- Common illness (acute) visits are subsidised for Blue and Orange cardholders, and for Merdeka and Pioneer Generation; Green is not subsidised for common illness.
- Dental is available to Blue, Orange, Merdeka and Pioneer Generation; from 1 October 2025 selected preventive dental procedures were extended to CHAS Orange. CHAS Green is not eligible for dental.
Healthier SG enrolment
Healthier SG pairs each resident with a regular family doctor to manage health over the long term through a personalised Health Plan. Singapore Citizens and Permanent Residents aged 40 and above can enrol.
- Enrol via the HealthHub app by choosing a participating clinic and booking your first, fully subsidised Health Plan consultation; in-person enrolment is also available.
- Enrolment waives the 15% MediSave co-payment for chronic treatment at your enrolled clinic (the S$500/S$700 limits still apply).
- Enrolled patients gain access to more affordable chronic medications and to subsidised screenings and vaccinations.
Screenings and vaccinations
Healthier SG includes nationally recommended preventive care, fully subsidised for eligible enrolled Singapore Citizens.
- Screenings include cardiovascular checks (blood sugar, blood pressure, cholesterol) and the recommended cancer screenings: cervical, colorectal and breast.
- Vaccinations under the National Adult Immunisation Schedule, such as influenza and pneumococcal, are available at the enrolled clinic.
- Eligibility for each screening depends on age, sex, existing conditions and when you were last screened.
How to make the most of these schemes
The pieces fit together best when used as a system. Choose a regular clinic and keep your visits there so your records, repeat prescriptions and screening reminders stay in one place. Bring your CHAS card and Singpass details to each visit, and ask the clinic to confirm what your MediSave claim and any subsidy will cover before treatment, so there are no surprises. If you have more than one covered condition or a recognised complication, mention it, as this can affect which annual limit applies.

Enrolment usually starts with selecting a participating clinic and booking a first Health Plan consultation, which is fully subsidised for eligible residents. From then on, chronic treatment at your enrolled clinic has the 15% MediSave co-payment waived, though the annual withdrawal limits still apply.
