Quick answer. MediFund is a Government endowment fund that acts as the safety net of last resort for needy Singapore Citizens who still cannot afford bills at public hospitals, community hospitals and approved long-term-care providers after government subsidies, MediShield Life and MediSave. You don’t apply online or by form – a Medical Social Worker (MSW) at the treating institution assesses your circumstances and refers the case to that institution’s independent MediFund Committee, which decides the amount. Two variants exist: MediFund Silver (aged 65+) and MediFund Junior (children). Other help includes the Medication Assistance Fund, IDAPE, AIC schemes and ComCare.
Singapore’s health-financing system is built so that the layers – subsidy, MediShield Life and MediSave – cover most bills. MediFund and the schemes around it are what catch the people those layers still leave short.

MediFund: what it is and who qualifies
MediFund is a Government endowment fund – only its interest income (not the capital) is distributed to MediFund Committees to help needy patients. To qualify you must be a Singapore Citizen, a subsidised patient, treated at a MediFund-approved institution, and facing genuine financial difficulty with the remaining bill after other help. It covers bills at public healthcare institutions, community hospitals and approved long-term-care facilities. There is no fixed assistance amount – the quantum depends on your and your family’s financial, health and social circumstances and the size of the bill.
How to apply: the Medical Social Worker route
You do not apply online or through a general form. Instead, you or your family approach the Medical Social Worker at the institution where treatment is being received. The MSW carries out a comprehensive assessment and makes a recommendation; the institution’s independent MediFund Committee then approves the assistance and decides the amount. MSWs can also connect you to other financing options – the Medication Assistance Fund, ComCare or charitable funds – if MediFund alone is not enough.
MediFund Silver and MediFund Junior
MediFund Silver (introduced 2007) lets any needy Singaporean aged 65 or above apply, better targeting the elderly. MediFund Junior provides targeted assistance for needy children. Both run through the same MSW → MediFund Committee process as core MediFund.
IDAPE: for seniors not on ElderShield or CareShield Life
The Interim Disability Assistance Programme for the Elderly (IDAPE) helps severely disabled elderly Singaporeans who could not join ElderShield when it launched in 2002 (too old, or a pre-existing disability). Eligible applicants must be Singapore Citizens in the relevant birth cohorts, need full help with at least 3 of 6 Activities of Daily Living, be assessed by an MOH-accredited assessor, and not be a CareShield Life/ElderShield policyholder or claimant. IDAPE pays S$250/month (lower-income) or S$150/month (middle-income) for up to 72 months, applied through the Agency for Integrated Care (AIC). See also CareShield Life & ElderShield.
Help with medication and devices
- Medication Assistance Fund (MAF): subsidises specified high-cost drugs on the MAF list for eligible subsidised patients – up to 75% for Citizens and up to 20% for PRs, means-tested by per-capita household income. The broader Standard Drug List also subsidises common drugs at public institutions.
- Seniors’ Mobility and Enabling Fund (SMF): subsidises mobility and assistive devices and home-healthcare consumables for eligible Singaporeans aged 60+ on lower incomes, administered by AIC.
- ComCare (Ministry of Social and Family Development) provides broader social assistance for low-income Singaporeans, often surfaced by the MSW.
How this fits with the rest of the system
MediFund is the last layer. Before it, government subsidy, MediShield Life and MediSave settle most of a bill – see how to pay for surgery for the full order. If you are worried about affording treatment, the single most useful step is to ask to speak to a Medical Social Worker early, ideally before or during admission, so all the available help can be arranged together.
